All participants are endowed with all of the rights, privileges, and program offerings generally accorded to or made available through any PCM administered program. By enrolling in classes, lessons, workshops, special events, and ensembles at The Phoenix Conservatory of Music, (all students, or their parents if under 18, must agree to the following), students and parents agree to abide by the following: ? Classes are $10.00 per week. Current class schedules and descriptions can be found online at www.PCMrocks.org ? Private Lessons are $40.00 per hour and $20.00 per half hour. ? There is a one time $10.00 Registration Fee. ? If payment for any class, lesson or event is made by check or credit card and the bank refuses payment, you will be charged a $20.00 processing fee to redeposit the check or to collect the funds in addition to the amount due. ? Accompanists are $30.00 per hour for rehearsals and $20.00 per performance. (Additional charges may apply for pieces of extreme length.) ? The Phoenix Conservatory of Music is a not for profit 501 c 3 organization. Tax Deductible donations can be made at the PCM Facility, via mail, or online at www.PCMrocks.org. PCM Comment Cards will be available in the Lobby they can be signed with contact information for follow up or anonymous. Please feel free to write suggestions, good testimonials, or give us ways we can make your experience better! Lessons ? By purchasing lessons, you are reserving your teacher?s time. The teacher is committing time from his/her schedule that can be spent elsewhere if not with you. We want to ensure the best experience possible for out students and our teaching artists. ? You will be given a set number of lessons based of off the number of lessons that you have purchased and you will create a schedule with your teaching artist that accounts for the number of lessons purchased. Once this schedule is set; students are committed to attendance and tuition for the duration of the schedule. ? To report an emergency absence, please call the PCM administrative offices at 602.353.9900 or 602.997.9915 and leave a message. You will need to call your teaching artist to let them know as well. If the teaching artist has to cancel, they will also call the PCM administrative office, and you, the student, directly. ? If there is an emergency, from the student or teaching artist, then the teaching artist, if their schedule permits may reschedule within 2 weeks. ? If their schedule does not permit, please contact the PCM administrative offices to reschedule with another teacher. ? It is the student?s (*and parent?s) responsibility to schedule make-up lessons. Even though your instructor will make every effort to assist you, do not expect them to keep track for you. ? If the make up lesson is not made within two weeks of the missed lesson, the lesson is forfeit. ? Lack of proper notice (a phone call within 24 hours of the emergency miss) automatically forfeits the scheduled lesson. There will not be a make up lesson. ? All lessons will be schedule no less than one week from the date of registration and payment. Payment for lessons must be made in full for the lessons to be scheduled. If a teaching artist teaches an unscheduled lesson (with out going through the PCM Admin. Offices and Scheduling One week from registration and payment date) and payment is not secured via the PCM drop box; then the lesson is forfeit, and the teacher will not be paid. ? Private lesson students must give two weeks notice from the end of their paid lesson times of their intention to continue or discontinue with their course of study to their private lesson instructor. If you intend to discontinue for any reason, please contact the administrative office at 602.353.9900. ? If you neglect to give notice of your intention to continue, your time slot may be re-assigned to a new student. ? It is the student?s responsibility to let your instructor know your intentions, especially if you wish to be considered for the same lesson day and time. ? Students will be required to sign in and initial the private lesson book in the PCM Lobby. Teachers must also initial to assure that the lesson has been held. Failure to secure a student?s sign in and a teaching artist?s initial will result in the teaching artist forfeiting the lesson. ? As your paid lessons are used; a reminder to purchase more lessons will be given to the PCM student in the PCM student mailboxes. ? Instructors are required to abide by all PCM policies regarding tuition payment and instruction. Classes ? If you have an emergency absence and miss a class, you may make up the class if you are able to attend a similar class at the PCM facility by the end of the class session. If there is not a similar class, the missed class time is forfeit. ? Registration for classes is finalized 48 hours before the class is scheduled to start. If there is not a minimum enrollment of 4 students at that time, class will be canceled. ? Late registrations are taken if the class has sufficient enrollment to be held (a class minimum of 4). ? Late registrations or partial registrations will not be prorated. ? In the event that your PCM teaching artist has an emergency and will miss class, every effort to secure a sub will be made. If a sub is not available, a make up class will be offered. ? Classes must be paid in full at the time of registration. ? If a student shows up for class, and they have not registered, they must immediately fill out the registration form and make payment; turning both into the PCM Drop Box before participating in the class. Instructors are required to abide by all PCM policies regarding tuition payment and instruction. Accompanists ? If you need an accompanist, PCM can help you secure one. PCM accompanist fees are $30.00 per hour for rehearsals, and $20.00 per performance. You can pay PCM directly via fax, mail, or payment drop. Mail Boxes ? Private Lesson students will have a file folder or ?mail box? on the main table in the PCM lobby next to the sign in book and Payment Drop Box. Please check your mail box regularly for updated class lists, payment reminders, recital updates and reminders, and news about the organization and music community. ? The same information will be made available to the classes via your teaching artist who is facilitating classes. Withdrawal Policy ? All withdrawals, from classes and lessons, must be made using the PCM withdrawal form. Please call the PCM administrative office to obtain one at 602.353.9900. ? Submit the completed form to the Executive Director at: PCM, PO Box 1163, Litchfield Park, AZ 85340 or drop it in the PCM drop box. ? Verbal notice to the instructor is not sufficient notice. ? One week must notice must be given with the intent to withdrawal otherwise the lesson is forfeit. ? Stopping attendance is not a sufficient withdrawal. Not attending lessons without a one week notice of withdrawal is an automatic forfeit of lessons. Refund Policy ? Student?s withdrawing from lessons or classrooms will be encouraged to try another course of action; different instrument, teacher, class, etc. If the student still wishes to withdraw, they will be expected to give at least one week notice. ? The student, if withdrawing immediately will be refunded the amount due to them minus the one week of lesson for withdrawing with out notice. ? The $10.00 Registration fee is non-refundable. ? No refunds will be given for classes after the third week. ? There are no refunds of donations, special event purchases, workshop or master class tuition. ? Students who have been dismissed by means of a faculty initiated dismissal with just cause; approved by the Executive Director are not eligible for refunds. Grievance Policy ? Students or Parents who feel they have a significant cause for a grievance are to request an interview with the Executive Director by calling 602.353.9900 or emailing info@phoenixconservatoryofmusic.org ? The situation will be documented, along with an action plan to resolve the situation signed by all parties. ? All grievances will be kept confidential as possible, handled with fairness to all parties, and resolved as quickly as circumstances will allow. Payment Policy (please see 6. Tuition, Fees, and Payment Schedules) Facility Use ? The PCM facility must be left as you found it. Please pick up and throw away all trash, keeping our campus welcoming and clean. ? PCM resources are for student and teaching artist use in house only. Books are available to read and research from at the Facility, not for check out. ? The PCM Facility is a non-smoking facility; inside and out. ? Parking is available on the South and North Side of the building. ? Student and Teaching Artist entrance is through Suite C on the South Side of the Building. ? If a student, family member of the student, or teaching artist causes damage to the building based off of misuse or neglect; the responsible party is liable for all repair costs incurred. ? Areas marked for Staff only are off limits to teaching artists and students of PCM. Program Change ? PCM Teaching Artists may suggest a program or instrument change for the benefit of the students and will council the student, and parents, to assist them with the change. ? If a change is made, all remaining tuitions will be applied to the new program. ? Students who wish to initiate a change may request counseling assistance if they feel a change is appropriate. Photo Release ? PCM Teaching Artists and Staff routinely take video footage and photos. These will be used to solicit funds through grants, foundation requests, corporate sponsorships, on the website and through media places like u-tube, myspace, etc. By enrolling in PCM photos, you release your or your child?s right to be videoed and photographed, and the photos to be used as mentioned above. If for any reason, you or your child cannot be photographed and videoed, we will need a written notice mailed to: PCM, PO Box 1163, Litchfield Park, AZ 85340 or put in the PCM drop box. ? We ask that parents and students who wish to have their personal pictures and videos shared with the PCM community submit them to the PCM administrative offices! Volunteerism ? Students of age and parents are encouraged and strongly advised to volunteer. There are many volunteer opportunities available depending on your talents and time available. Every minute you give makes the organization stronger, and let?s music touch more people?s lives! We are only as strong as our community support. Ways you can help are: 1. Special Event Committee 2. Restaurant Fundraising Night Committee 3. Employee Giving Committee 4. Working Concessions at the Cardinal?s Games. 5. Working the 50/50 raffle at the Phoenix Coyotes Hockey Game 6. Helping Out in A Classroom on site and in our outreach programs. 7. Office Assistance with keeping our resource room well stocked filing, stuffing mailboxes, etc. 8. Helping to write grants or garner corporate sponsorships. 9. Having donation parties for PCM 10. Website maintenance and design 11. Being an advocate and asker for the organization! Student Code of Conduct (please see 8. Student Code of Conduct) Liability Waiver ? All students and parent/guardians by signing up for PCM programs agrees to the following liability waiver: 8. Student Code of Conduct PCM Students, Teaching Artists, Faculty and Staff are expected to conduct themselves in a fashion that supports the efforts of the organization, including: 1. Attending lessons on time. If you arrive late do not expect your instructor to alter their schedules. You will receive full attention for the time remaining in your scheduled lesson. 2. Preparing Assignments dictated by your instructor. Failure to practice or come to your lesson prepared, or failure to bring required supplies, or purchase the necessary lesson materials in a timely manner may result in a forfeited lesson. Repeated offences may result in dismissal. No refunds are given for students dismissed on these grounds. 3. Participating in performances, ensembles, classes, and workshops as prescribed by your instructor. Failure to participate could have disastrous consequences on other participants. If you cannot participate, inform the instructor at the time the assignment is offered. If you accept the assignment and fail to participate, you will be issued a warning. Repeat offenses may be grounds for dismissal. 4. Respecting the needs of others and refrain from rude, distracting behaviors in private lessons, classes, ensembles, performances, or workshops. Disruptive behavior is grounds for immediate dismissal without warning or refund privileges. 5. No food or drink besides water in the private lesson studios or classrooms. Water must be in closed containers. 6. Parents are generally expected to wait outside the private lesson studio to allow students to fully concentrate on the teacher?s instruction. The parent may stay for the first class if the student feels uncomfortable in new surroundings; or if the teacher permits it for the well being and better practice habits of the student. Early childhood parents are encouraged to stay in the classroom. 7. Young children must be accompanied and supervised by parents at all times when not in a classroom or lesson studio. 8. Persons attending PCM activities, at any location, are to confine themselves to the building and parking lots assigned for their use. 9. PCM personnel may require the assistance of students and parents in returning rooms to their state prior to the PCM event, or to return instruments and gear to their proper place. 10. Please do not attend the lesson if you or your children are ill. This is an emergency absence. 11. If you or your child should become ill during a class or lesson, we ask that you return home immediately. This is for the protection of our other students, staff, and faculty. 12. Parents must pick up their students on time. Instructors have tight schedules and cannot watch your child for you. Remember that when not in the lesson room, your children are your responsibility. 13. PCM and all associated properties have a policy of no smoking, no illegal substances, or weapons within 100 feet of the PCM property. There is no smoking anywhere on the PCM campus. 14. PCM reserves the right to discontinue any student whose work or behavior is unsatisfactory, who is harassing teaching artists or other students, frequent absences, tardiness, behavior problems, lack of interest or progress, failure to respond to a request in a positive way, or failure to abide by policies and codes of conduct. Faculty may initiate a student dismissal when warnings, parental conferences with the teaching artist, and then a follow up with the student, parent, teaching artist, and Executive Director have proven ineffective. Anyone who is caught in the act of or proven guilty of theft or property damage are subject to immediate dismissal and prosecution.
